What is Basti Chikitsa?

Basti therapy, also known as Ayurvedic enema therapy, is one of the main treatments in Ayurveda. It involves the administration of medicated herbs or oils into the rectum to cleanse and balance the doshas, especially Vata.

Why is Basti Chikitsa Recommended?

Vata dosha governs all movement, nerve impulses, and nervous system functions in the body. The colon (Pakvashaya) is the primary seat of Vata. Basti directly treats Vata at its root cause.

Therapeutic Mode of Action

  • Preparation: Light diet or fasting; patient positioned comfortably on the left side in a calm, hygienic environment.
  • Procedure: Selected herbal decoction or medicated oil is prepared fresh and introduced gently into the rectum using sterile clinical apparatus.
  • Retention & Expulsion: The patient retains the liquid for a prescribed duration to allow colonic absorption before natural expulsion.
  • Post-Treatment: Rest, hydration, and restorative Vata-soothing diet.

Clinical Indications

Basti Chikitsa is prescribed following diagnostic evaluation for patients presenting with:

Osteoarthritis, Sciatica & Lumbar Spondylosis
Chronic Constipation, Gas, Bloating & IBS
Anxiety, Depression & Neurological Nerve Pain
Asthma, Bronchitis & Sinusitis
Joint Stiffness, Muscular Pain & Atrophy
Gynecological & Reproductive Health Issues

Three-Phase Procedural Protocol

Phase I

Purva Karma (Preparation)

  • Fasting or light dietary preparation before therapy.
  • Localized Abhyanga oil massage of lower back, abdomen, and legs.
  • Nadi Swedana (herbal steam) over abdomen and lumbar regions.
Phase II

Pradhan Karma (Primary Procedure)

  • Niruha Basti (Decoction Enema): Cleansing enema prepared with herbal decoctions, honey, salt, and oils for Vata detox.
  • Anuvasana Basti (Oil Enema): Nourishing, lubricating enema using medicated oils to soothe Vata and build strength.
Phase III

Paschat Karma (Post-Therapy Care)

  • Rest in a comfortable environment and warm light diet.
  • Adequate hydration to support systemic toxin elimination.

बस्ति कर्म (Medicated Enema Therapy) — Clinical Documentation

Basti is classically revered as the half of all Ayurvedic therapeutics (*Ardhachikitsa*). Because the colon is the primary site where Vata dosha accumulates, rectal administration of medicated lipid and decoction preparations directly reaches the systemic circulation through colonic mucosal absorption.
